05 · Diagnosis plus dynamics

Psychiatry

Diagnosis names a clinically important pattern; dynamics asks how this person’s pattern persists, shifts and responds.

A dynamical overlay supplements diagnosis and evidence-based care; it does not replace either.

The human question

What must a formulation know that a symptom list cannot show?

What this family holdsRegimes, formulation, dynamical taxonomy, timescales, multimodal care, individual trajectories and navigable complexity.

Psychiatric condition as a dynamical regime

Symptoms may cohere because a whole pattern of movement has reorganized—not because one component broke.
In lived experience

The person is not a regime. A regime is a pattern their life is moving through—and patterns can have histories, thresholds and exits.

Definition

A dynamical regime is a recurring organization of states, transitions and responses. Depression, trauma, addiction or psychosis can be studied partly through persistence, switching, variability, coupling and recovery after perturbation, alongside conventional symptoms and diagnoses.

Flow Hijacked lens

The framework asks not only what is present, but how it moves: what keeps the state alive, what makes switching likely, what amplifies perturbation and what restores navigability. Diagnosis remains important for evidence and safety; dynamics add a second layer.

FH / DEFINED TERM

Clinical-dynamical formulation

Map the present state, likely transitions, slow landscape, amplifiers and available inputs around one person.
In lived experience

A good formulation should make care more precise and the person less reduced—not simply replace one label with mathematical vocabulary.

Definition

Lecture 24’s Flow Map supplements diagnosis with eight questions: accessible state-space, attractor structure, compression, perturbation-amplification geometry, transition structure, network organization, timescales and candidate control inputs. It is a disciplined way to organize hypotheses rather than a score.

Flow Hijacked lens

Formulation separates observation from inference. “Three nights of little sleep” is an observation; “approaching mania” is a hypothesis requiring history, examination and differential assessment. The map is revised when response, context or new evidence disagrees.

Dynamical taxonomy

Classify the deformation that needs changing, not only the symptom cluster that names it.
In lived experience

Two people can share a diagnosis and need very different routes because their systems are stuck, amplified or depleted in different ways.

Definition

Lecture 25 proposes profiles such as compression-dominant, attractor-trap, amplification-dominant, transition-instability, prediction-rigidity, prediction-destabilization, network-overcoupling and allostatic-depletion. A person may show several profiles across different timescales.

Flow Hijacked lens

The same diagnosis can contain different dynamics and therefore different first treatment logic. One depression may need behavioral reopening; another medical assessment and reserve restoration; another protection from unstable switching. The taxonomy aims to sharpen questions, not manufacture certainty.

State, trait and timescale separation

What changes in minutes is not the same object as what changes across months—even when they interact.
In lived experience

Feeling calmer today can be genuine progress without proving that the slow vulnerability has already resolved.

Definition

Acute arousal, intoxication or panic evolve quickly; sleep architecture, plasticity, habit, identity, medication exposure and relationship change often evolve more slowly. Separating timescales clarifies why a state can improve while vulnerability remains, or why a slow intervention has no immediate visible effect.

Flow Hijacked lens

A coherent plan matches input to clock: crisis stabilization for the fast layer, sleep and medication review for physiological rhythms, repeated therapy for learned parameters, community and role change for the social landscape. Timing is part of mechanism.

Treatment as multimodal control

Medication, psychotherapy, body, relationship and environment enter one system through different channels and clocks.
In lived experience

Good care is often orchestration: enough physiology, safety and support for difficult psychological work to become possible.

Definition

Multimodal care coordinates biological, psychological, behavioral, relational and environmental inputs. A medication may lower gain or stabilize mood; sleep may restore reserve; psychotherapy may organize learning; social change may remove cues or create belonging. Inputs can prepare, reinforce or interfere with one another.

Flow Hijacked lens

The question is not which modality wins, but which sequence fits the current deformation, urgency, reserve and preference. The best intervention can be the wrong first intervention; an apparently modest preparation can make a later evidence-based treatment usable.

Idiographic measurement and the ergodic problem

Group averages and one person’s changing dynamics answer different questions; responsible care needs both.
In lived experience

The most useful signal may be not “How bad was this week?” but “What changed before the difficult hours, and how long did return take?”

Definition

Idiographic modeling uses dense within-person observations—experience sampling, sleep, behavior, physiology or narrative—to study temporal patterns. The ergodic problem warns that relations across people need not equal relations within one person over time.

Flow Hijacked lens

Flow Hijacked favors trajectories over snapshots: recovery time, recurrence, switching and context may reveal what a symptom total misses. Yet individual data must remain anchored to group evidence, measurement quality and clinical interpretation rather than becoming evidence-free personalization.
